ARRANGING CARE

Arranging care.
What to expect.

You don’t need to know exactly what care looks like before you contact us. We can start with a conversation about your situation.

FROM ENQUIRY TO YOUR FIRST VISIT

What happens
after you contact us?

Care starts with understanding your needs. These are the steps we will discuss with you, subject to assessment and availability.

01

Your first enquiry

Tell us where you live, the help you are looking for and when you would like support. Your local team will discuss availability and explain the next steps.

02

An initial assessment

We discuss daily routines, health and mobility needs, preferences and what you want to achieve. You can involve a relative or someone you trust.

03

Your care plan

Together, we agree the tasks, visit arrangements and preferences to include in your plan. We discuss what we can provide before care starts.

04

Introducing your carers

We consider your needs and personal preferences when matching carers. Your team will explain introductions and how to prepare for the first visits.

05

Keeping in touch

Tell your local team how care is working for you. We review support as needs change and explain how to raise questions, feedback or concerns.

PREPARING FOR YOUR CONVERSATION

What would you
like us to know?

You may find it helpful to think about the routines you want to maintain, the tasks that feel difficult and the people you would like involved in decisions.

  • The kind of help you are exploring.
  • Your preferred days, times and routines.
  • Any cultural, social or religious preferences.
  • Whether you are planning ahead or need support soon.
  • The questions you would like to ask.

You do not need to send detailed medical information through our initial website form.
